Registration Assistance Forms
Before the completion of any form please confirm that the appropriate prerequisite courses have been completed. To see a course's prerequisites, go to 'Registration' within 'Self Service' and search for the course. Click the course name under 'Title'. Click on the 'prerequisites' square on the left side under 'Class Details'. This page will give you a list or pre-requisites and other restrictions on enrollment.
Request for Authorization Form

- Indicate CRN, Course Number and Title, i.e. 10015, ART 1201, 2D Design Foundations
- ONLY use your FAU email address
- Complete the form and Submit
- You will receive notification via your FAU email address
Directed Independent Study Contract

- Print and fill out contract completely
- Have professor review, sign and return to you.
- Return signed contract to the Art Office (AH 118) on the Boca Campus or via email: art@fau.edu
