Registration Assistance Forms

BEFORE YOU COMPLETE ANY FORM PLEASE CHECK TO SEE THAT YOU HAVE TAKEN THE APPROPRIATE PREREQUISITE COURSES  (go to the schedule and click on the CRN or Course Registration Number, which will give you a list or pre-requisites and other restrictions on enrollment).

Request for Authorization Form 

    • Indicate CRN,course number and title, i.e. 10090, ART 1201, Design
    • Use ONLY your FAU email address
    • Complete the form and Submit
    • You will receive notification via your FAU.edu email address

Directed Independent Study Contract 

    • Print and fill out contract completely
    • Have professor review, sign and return to you.
    • Return signed contract to DEPARTMENTAL OFFICE BOCA CAMPUS AH 118 or via email: art@fau.edu
