Change of Instruction

 

Eligibility:

1.	You have used your GI Bill® Benefits before
2.	Transfer Student
3.	Major/Program Changes
4.	Returning to FAU after NOT using benefits for over 1 year

 

If one of the above qualifications applies to your situation, complete the following forms with Florida Atlantic University:

1.	Complete the VA Certification Request  Form.
2.	Complete Form 22-1995 - Request for Change of Program or Place of Training. On the top of this form, please write on the top of this form that you are applying for Chapter 1607 benefits.
3.	Provide a copy of the DD-2384 - Notice of Basic Eligibility.
4.	Provide a copy of Certificate of Eligibility from the VA.
5.	Provide a copy of the DD-214 MEMBER 4.
6.	Prior Education and Training Verification Form .
