Dementia Care Navigation (DCN)

FAU's Schmidt College of Medicine's Dementia Care Navigation (DCN) program addresses the unmet needs of those affected by Alzheimer's Disease and other related dementia (ADRD). The program offers a holistic plan of action and solutions for ADRD patients, caregivers, families, and communities to address wide-ranging needs and achieve the best health outcomes.
