Immunizations and Insurance Compliance
Pursuant to Florida Board of Governors Regulation 6.001(9), prior to registration, each student accepted for admission at Florida Atlantic University must submit a signed FAU Immunization Form. Florida Atlantic University requires documented proof of immunizations to Measles and Rubella. In addition, pursuant to Florida Board of Governors Regulations 6.007, effective July 1, 2008, all new matriculating students must also provide documentation of vaccinations against Meningococcal Meningitis and Hepatitis B or provide a signed waiver for each declined vaccination.
In order to be considered official, the Immunization Form must contain an authorized signature AND an office stamp. In lieu of an authorized signature and stamp, copies of official records may be attached to the Immunization Form. Official records must include the student’s name and front cover of all documents. We reserve the right to interpret the validity of all documents.
All students must complete the demographic information at the top of the form and sign in Section D.
Students under the age of 18 must also have a parent/guardian sign in Section D.
Students born after 1956 must have Sections A and B completed by a physician if official immunization documents are not attached to the form.
Students (or their parent/guardian if the student is under the age of 18) must sign in Section B if they do not have documentation of vaccination against Meningococcal Meningitis and/or Hepatitis B and elect to waive either or both of those vaccines.
Proof of immunization is required prior to registration.
FAU may have received some immunization information electronically if you have attended a Florida public high school. In addition you will need to complete the FAU Immunization Form and provide any missing information.
To submit the FAU Immunization Form and additional immunization documentation, please complete the following steps:

- If you need a medical provider to complete the form, the form can be submitted via:

You may be able to obtain your immunization records from:
- A previous university
- Your pediatric doctor
- Copies of other official immunization documentation (e.g., Florida Certification of Immunization, immunization booklet, Florida high school transcripts)
- NOTE: You are required to submit the signed FAU Immunization Form in addition to any of the above documentation.
