Request Services from the Center for Autism and Related Disabilities (CARD)

IMPORTANT! Please read carefully before beginning the registration process.

Documentation of Diagnosis

Florida Atlantic CARD kindly requires proof of an autism diagnosis (or a related disability) for children or adults before registration can be completed. A list of accepted diagnoses is below:

  • Autism Spectrum Disorders (Levels 1-3)
  • Autistic Disorder
  • Asperger Syndrome
  • Developmental Delay (DD)
  • Pervasive Developmental Disorder Not Otherwise Specified (PDD-NOS)
  • Rett Syndrome
  • Childhood Disintegrative Disorder (CDD)
  • Dual Sensory Impairment or Deaf-Blind
  • Sensory Impairment with another disabling condition

To help us process your registration, please provide one of the following documents:

  • The cover page of an IEP that lists Autism Spectrum Disorder (ASD) as the educational eligibility
  • An evaluation report that clearly shows the individual’s name and the autism diagnosis
  • A letter from a doctor or qualified evaluator confirming the autism diagnosis

Proof of Guardianship

Additional documentation may be required for individuals 18 years of age or older.

If the adult has a legally appointed guardian

The legal guardian must complete and sign the registration form and upload court documentation verifying the guardianship appointment.

If the adult is 18 or older and does not have a legal guardian

The adult must complete and sign the form themselves. They may choose to include contact information for a family member or supporter if they would like someone else involved in communications.


Before you begin the online registration, please make sure you have the applicable documents ready to upload.

Please note: Registration cannot be completed without uploading the required documents. The system does not save your information, so you will not be able to return later to finish the form.
