SOAR Nomination Forms
Please select the award nomination form you want to fill out by clicking the name of the award. When you are taken to the nomination form, you will also have the chance to read more about the award. If you have any questions, please contact COSO on your respective campus.
*Nominations are due March 13, 2015*
Instructions for submitting nominations for all SOAR Awards:
- Any FAU student, faculty, or staff may nominate.
- Please complete the appropriate form for the category in which you are submitting the nomination (you may submit nomination forms for as many different categories as you like, but please do not submit more than one form per individual category.)
- Make sure that contact information is accurate and complete.
- To be eligible, a nominee must be a current student, employee at FAU, or a current registered student organization in good standing. A student must be enrolled in an undergraduate or graduate degree program at the time of their nomination.
- All nominations must be submitted by Friday, March 13, 2015
