This website allows you to report information about concerning behaviors or incidents in our FAU community. Please read the description of each category and choose the appropriate report by selecting the corresponding link for the issue you wish to report.

Florida Atlantic University supports a standard of care, safety, security, and inclusion to provide the best educational and work environment possible. You can help FAU maintain this standard by reporting conduct violations, sexual misconduct, discrimination or harassment, general concerns for community member well-being, ethics issues, or other potential issues or threats to the FAU community.

SEXUAL MISCONDUCT / TITLE IX

Go to the Sexual Misconduct or Gender Discrimination Report Form (Title IX) 

Report incidents of sexual misconduct. Incidents include, but are not limited to:

  • Dating Violence
  • Domestic Violence
  • Forcible Rape
  • Forcible Sodomy
  • Forcible Fondling
  • Forcible Sexual Assault with an Object
  • Incest
  • Stalking
  • Statutory Rape
  • Sexual Harassment
  • Gender Discrimination
  • Retaliation based on prior sexual misconduct report

DISCRIMINATION / HARASSMENT (non-sexual)

Report incidents of discrimination and/or harassment (not included as sexual misconduct above) based on:

  • Veteran Status

STUDENT BEHAVIORAL CONCERN

Go to the Incident Reporting Form

Report behavior that is concerning. Reportable behaviors may include, but not be limited to, a student who you feel may be struggling with physical or psychological issues, is in need of support to maintain appropriate behavior in academic areas or displays any other behaviors of concern that you would like to see addressed or would like to discuss.

STUDENT CONDUCT ISSUES

Go to the Incident Reporting Form

Report academic integrity or social behaviors that you may feel are violations of the Student Code of Conduct. Behaviors may include any activities or actions that you feel are an infraction of the community standards. The Student Code of Conduct (University Regulation 4.007), which provides details related to all community standards, can be found in the Student Code of Conduct. Please note that violations of alleged sexual misconduct should be reported via the SEXUAL MISCONDUCT / TITLE IX link on this page.

HAZING

Go to the Incident Reporting Form

Report concerns related to hazing. No student organization, student club, including fraternity or sorority chapter, colony, or student member of such chapter or colony shall conduct, engage in, or condone hazing activities. This policy shall apply to acts conducted on or off campus whenever such acts are deemed to constitute Hazing. Hazing includes, but is not limited to, any action or situation that recklessly or intentionally endangers the mental or physical health or safety of a student for purposes including, but not limited to, initiation or admission into or affiliation with any organization operating under the sanction of Florida Atlantic University.

GENERAL STUDENT CONCERN

Report general student concerns that you would like to be addressed by the Dean of Students Office.



BEYOND FOOD PROGRAM and FOOD PANTRY

Go to the Reporting Form > 

Report a concern related to food insecurity. Students may use this form to self-report needs and seek resources from the Beyond Food program.  Faculty, staff and community members may  also refer students that are hungry or in need of basic hygiene products.  Students will be provided with up to a 3-day supply of food and/or personal products (e.g. soap, shampoo, etc).   This program provides students with various resources to assist in raising food security levels in order to support holistic well-being and academic success.

WASTE, FRAUD, AND ABUSE

Report allegations of waste, fraud and abuse within the University. 

Concerns regarding waste, fraud and abuse may be reported through the anonymous complaint form on the FAU Inspector's General's Website, or through FAU's third party-ethics hotline at www.lighthouse-services.com/fau.

OFFICE OF CIVIL RIGHTS AND TITLE IX – APPEAL FORM

Go to the Office Of Civil Rights And Title IX - Appeal Form

All parties involved in an OEI investigation  can  appeal a determination  regarding  responsibility, a dismissal of a formal complaint, or a dismissal of any allegations of a complaint Appeal Forms must be  submitted  within five (5) business days of the dismissal or determination.   

Appeals will only be accepted on the following basis:  

  1. Procedural irregularity that affected the outcome of the matter; and/or  
  2. New evidence that was not available at the time of determination regarding responsibility or dismissal that could affect the outcome of the matter; and/or 
  3. The Title IX Coordinator, investigator(s), or decision-maker(s) had a conflict of interest or a bias for or against complainant(s) or respondent(s) generally or the individual complainant or respondent that affected the outcome of the matter.

ETHICS CONCERN
