Report of Outside Employment/Professional Activity

As we begin the new fiscal year, HR would like to remind all full time employees that University policy requires that anyone engaged in outside employment/professional activity during the new fiscal year (July 1, 2017 – June 30, 2018) must report these involvements. This is to ensure that participation in these activities does not create a conflict of interest with one’s primary duties here at FAU. 

Before assuming or continuing any other interests outside of FAU please complete the “Report of Outside Employment or Professional Activity” form(s); located at:   http://dorsrv2.fau.edu/DORForms/FormReportOfOutsideEmployment.aspx

In the event University equipment, services or facilities is used in the course of the activity, please complete the “University Equipment, Services and Facilities” form located at:  http://www.fau.edu/hr/files/Equipment.pdf
