After completing this Web page and clicking on the submit button, an email will be sent to the supervisor specified in the space below. Supervisors will need to review the information and submit their approval to Denise Campbell at dcamp@fau.edu by replying to the email.

Select the role the user will be performing in the EPAF system:
Originator – can only create new EPAF transactions, but cannot approve EPAF records.
Approver or Proxy – can approve or acknowledge EPAF transactions.
Super User – reserved for Human Resources

Select the appropriate approver level – for Approvers Only
