Fall 2020 Return-to-Campus #3 - COVID-19 Training and Wellness Attestation

As the Fall 2020 semester begins, all members of the university community must complete COVID-19 prevention training. Students must complete a Wellness Attestation/Check-In form twice per week. Employees must complete the attestation form prior to the opening of the semester and then twice weekly once they are recalled to full- or part-time work on campus.

COVID-19 Training
Employees and students will be presented with the option to complete the training titled
COVID-19 Required Training (Employees)
and
COVID-19 Required Training (Students)
when logging in to any system that requires a NETID on campus.
The training takes approximately 30 minutes
to complete.
Employees and students will be able to bypass the training for three log-ins using the “Finish Later” option, but ultimately will not be able to proceed without completing the training. Everyone is encouraged to complete the training at their earliest opportunity.
Attestation for Employees
Employees must complete the attestation form prior to the opening of the semester. Thereafter, an employee must be identified by their supervisor (see next section) as coming to campus in order to start the second phase of the attestation process. Once the supervisor indicates in Workday that the employee should return, the employee will receive an automated email notifying them they must complete the Wellness Attestation/Check-In prior to returning to campus, and twice per week, thereafter.
The attestation will appear whenever the employee attempts to log-in to any major University system, such as
Workday

Canvas
or
Banner
.
Employees will be presented with a series of symptoms and conditions they need to answer. At the end of the process, the employee will be approved to return to campus or instructed to remain home, and that an FAU healthcare representative will contact them. In addition, there is an express form for subsequent days if the employee remains in good health.
Supervisors
A job aid is available in Workday to help guide supervisors through this process. This has two options: 1) if supervisors are identifying 10 or fewer employees to return to campus; and 2) if a larger number of employees are required to return to campus. For option 2, supervisors should submit the Z number of the employees via a
HelpDesk
ticket.
Student Attestation
Beginning on the first day of classes, students will receive a Wellness Attestation/Check-In twice per week when attempting to log-in to any major University system.
Residential students who arrive before the start of classes will be added as they move into their residence halls.
