acknowledge the cost per mile is $0.445 per mile as shown on the cars odometer 
and that the release of any vehicle must be accompanied by this form.

Users are responsible to return the vehicle in the same condition as received, 
with a 3/4 (or full) tank, and at the same office where the vehicle was picked up.
