Student Health Services does not write notes excusing students from missed class(es). We do, however, offer two options:
-
A receipt of your charges can be printed to provide documentation of your visit.
-
An Authorization for Release of Medical Records can be completed for the class(es) missed, so your professor(s) can call the clinic to speak with the Nurse Practitioner to confirm your appointment and/or diagnosis.
You may request one of options above from the Student Health Services front desk.
