Who provides the services at Student Health Services? 
You will be seen by a state licensed Master’s prepared board certified Advanced Registered Nurse Practitioner.

Can anyone be seen by the Nurse Practitioner? 
No, only FAU students who are enrolled in the current semester are eligible for services. If you were enrolled in the previous semester and did not graduate you may be eligible for services. Inquire within the Clinic.

Can I just walk-in and be seen by the Nurse Practitioner? 
No, services at Student Health Services are provided by appointment only. You may call or stop by the front desk to schedule an appointment.

Do I need to bring anything to my appointment? 
Yes, make sure you have a photo ID with you when you arrive for your appointment. It can be your OWL card or Driver's License.

How much does it cost to see the Nurse Practitioner? 
An office visit is $5. Any charges for medications and/or lab work are in addition to the office visit charge. For a more complete list of costs please click here.

Do I need to pay for services at the time of my appointment? 
No, any charges will be posted to your student account. You can then pay online.

Can I be seen at the Jupiter clinic even if I don’t have classes on the Jupiter campus? 
Yes, as long as you are enrolled in the current semester and taking class on any one of FAU’s campuses (Boca, Davie, Sea Tech, Harbor Branch, or Ft. Lauderdale) you can be seen at the Jupiter clinic.

What do I do if I need to cancel or reschedule my appointment? 
You must call or stop in to cancel/reschedule your appointment by 5 p.m. the workday prior to the scheduled appointment to avoid being charged a “no show fee”.

$10 “no show fee” for general medical appointments
$20 “no show fee” for dental or medical appointments over 40 minutes

Can I get a copy of my medical records?
Yes, you may request a copy of your medical records from the clinic at any time; however, your medical records are confidential and require your written consent to be released. The only exceptions would be records subpoenaed by the court, or determined necessary for public welfare.  Fill out the appropriate form (click here) and submit it to the clinic by one of the following methods:

Is there a cost to get copies of medical records? 
Yes, there is a charge of $1.00 for the first page and $0.25 for each additional page for all copies made of medical records. This fee covers the labor and materials to retrieve and copy records, any fax toll calls, and/or postage costs.

No copy charges will be assessed when SHS refers for care in the community.

Can I get a “doctor’s note” for missing class? 
No, the clinic does not write notes excusing students from missed class(es). We do, however, offer two options:

1. A receipt of your charges can be printed to provide documentation of your visit.
2. An Authorization for Release of Medical Information can be completed for the class(es) missed, so your professor(s) can call the clinic to speak with the Nurse Practitioner to confirm your appointment and/or diagnosis.
