The Immunization Office is located in Room 114 of the Student Support Services Building (SU-80) south of the Student Union on the Boca campus.To obtain copies of your immunization records you may use the MyHealth Web Portal (https://myhealth.fau.edu).
Pursuant to Florida Board of Governors Regulation 6.001(9), prior to registration, each student accepted for admission at Florida Atlantic University must submit a signed FAU Immunization Form. Florida Atlantic University requires documented proof of immunizations to Measles and Rubella. In addition, pursuant to Florida Board of Governors Regulations 6.007, all new matriculating students must also provide documentation of vaccinations against Meningococcal Meningitis and Hepatitis B or provide a signed waiver for each declined vaccination.
The form consists of four pages and should be read carefully. All students must complete the demographic information at the top of the form and sign in Section D. Students under the age of 18 must also have a parent/guardian sign in Section D. Students born after 1956 must have Sections A and B completed by a physician if official immunization documents are not attached to the form. Students (or their parent/guardian if the student is under the age of 18) must sign in Section B if they do not have documentation of vaccination against Meningococcal Meningitis and/or Hepatitis B and elect to waive either or both of those vaccines.
