Please call the Delo Medical Associates office at (772) 871-5900 to schedule an appointment.  PLEASE NOTE:  Identify yourself as an FAU student and be sure to take the following items with you when you arrive for your appointment: 

1) Your OWL Card/student identification card
2) Your insurance card (if you have health insurance).  Dr. Delo is a UnitedHealthcare Choice Plus provider.
