2009-10 STUDENT INSURANCE PLAN
Please note: Enrollment for the Annual/Fall student insurance plan is closed.
Information for the 2009-10 student insurance plan is now available at the Insurance for Students website. Printed copies of the policy and application form will be available in the near future. A representative from Insurance for Students is in the International Students office (SU80, Room 214) on Thursdays from 1:00 p.m. to 4:00 p.m. Extended hours may be available during peak registration periods.
2009-10 FAU Student Injury & Sickness Insurance Highlights
- $250,000 maximum benefit per injury/sickness per policy year.
- $400.00 deductible (waived for services provided at FAU Student Health Center)
- 100% coverage at FAU Student Health Center
- Annual physicals, including annual pap smear, covered at FAU Student Health Center only
- $1,000.00 Rx benefit per policy year. Reduced co-pay at FAU Pharmacy.
- Benefits payable at 80% of benefit limits within the UHC Network of providers and 70% outside of the Network
- Collegiate Assistance program for nurse triage after hours service (1-866-281-9725).
- Dependent coverage available to enrolled students.
Open enrollment periods:
- Spring/Summer -- Ends on 2/09/2010
- Summer -- Ends on 6/17/2010
(Enrollment will not be allowed past the deadline).
