The SACS MUST Statement Review Form must be viewed through Microsoft Word. Double click on Must Statement Review Form to open. Choose the "Save As" option to save the form to a file on your hard drive. Select a name for the file and a folder/location in which to store it. When you are ready to post the form, it should be emailed as an attachment to moconnor@fau.edu. The form will be posted to the web under your committee name. The following is intended to assist you in using the form:
Upper right hand corner:
      Check In Progress if review of Must Statement not yet completed or
      Final if review is completed.
      Updated: Today's date.
Committee:
      Name of principal committee (i.e., Educational Programs)
Must Statement Number:
      Each Must Statement is numbered within its criteria section. Refer to the Must Statement
      Number for the correct criterion. (i.e., 4.2.1/1)
Must Statement:
      Text of Statement
Compliance or Non-Compliance:
      Indicate Compliance or Non-Compliance. Do not use the term "Partial Compliance."
      Complying partially should be classified as Non-Compliance.
Evidence of Compliance:
      Brief text statement of why determination of Compliance or Non-Compliance was reached.
Documentation:
      Cite all documents used to reach determination of Compliance status. Give specific titles,
      page numbers, dates (in the case of meeting minutes, memos, etc.) and website address.
Location of Documentation:
      Leave blank.
Action Needed/Taken:
      "Recommendation" must be made when Non-Compliance is determined.
Other Comments:
      Optional. You may wish to make a "Suggestion" if Compliance is met, but could be
      strengthened or a "Commendation" if you find an area of particular strength.
