• High School Non-Degree Residency Classification Form
  MUST BE TYPED ONLINE
  See information below for completion of the form.
  • Florida Resident Classification sections needs to be completed by the parent/guardian. Residency is based on the parent/guardian of the high school student and the information provided is based on parent/guardian.
  • Documentation is needed for all sections that are completed on the form.
  • Citizenship section -- MUST BE CHECKED -- If the parent/guardian or student is a resident alien, copies of the front and back of the Resident Alien card are required. Also indicate the country of citizenship on the copies.
  • Print form
  • Parent/Guardian must sign the Affidavit section and date.

• Immunization Form (See the information below for completion of the form).
  The FAU Immunization form cannot be typed online--print out pages and complete.
  • Please fill out the top portion of the form.
  • For Section A, B & C
    • If the student has copies of records that are acceptable (with the appropriate immunizations and physician's signature and office stamp); attach these copies to the immunization form.  In this case, the physician signature is not needed on the FAU Immunization form.  Also acceptable is high school printout of inoculations with high school stamp.
    • If the student does not have copies of records, then Sections A, B & C must be completed by a physician and also signed and stamped.
    • Student and parent signature is required is Section D.
