CLAST Exemption Form - Used when a student has attained exemption from either the math or English portions of the CLAST exam by alternative means. Complete and submit to a Business Undergraduate Studies office.

AA Degree Application - Used to apply for your AA degree (check FAU's academic calendar for due dates).
UNIVERSITY FORMS

Application for Admission to FAU - Used to apply to FAU's undergraduate degree programs.

Enrollment Verification Form - This is a Registrar form, allowing you to obtain documentation from the Registrar's office concerning your enrollment status at FAU.

Forgiveness Policy Form - FAU form that allows students to "forgive" prior grade on a repeated course.

Gordon Rule Evaluation Form - Used by the Registrar's office, to apply transfer work toward the state's Gordon Rule requirements.

Immunization Form - This Student Health form is for the required proof of immunization for all new FAU students.

Transient Student Form (use Transfer Services link) - Used for taking courses as a visiting student at other Florida universities and community colleges. This link is to the electronic version, allowing you to submit the form by web, and eliminating the need to hand-carry the form. The electronic form is usually processed in 1-2 working days. A paper version is available within the Freshman Academic Advising Services or Registrar's offices, if needed.
