Change of Instruction

1.  Download Transient Student Form, or

2.  Fill out the online Transient Student Form from Facts.org.

Download Transient Student Form:

By completing the Transient Student Form and upon approval, permission is granted to take courses at another institution. These courses will transfer to Florida Atlantic University and be applied to the prerequisites of your degree. You may download and complete the Transient Student Form or apply online at www.facts.org (See Online Transient Form instructions below).

INSTRUCTIONS:

Indicate which Institution you will be attending as a Transient Student.

Complete Section A of the Transient Student Form.  Sign and Date this form.

In Section B, indicate the courses you wish to take as a Transient Student.  Submit to your Academic Advisor for approval.  Your Academic Advisor must sign and date this section.  If this form is not completed in its entirety, it may not be processed or inaccurate processing may occur.

Section C is completed by the Florida Atlantic University’s Registrar’s Office.

Important:  Submit the approved Transient Student Form to the VA Office of the other institution in order for you to receive proper credit.  Provide Florida Atlantic University with a copy.  Please submit Florida Atlantic University’s copy to the following location below.

Note:  If you are taking classes at FAU and another institution also submit a Certification Request Card to FAU.  If you are only taking classes at an institution other than FAU, then you do not have to complete a Certification Request Card with FAU.  Additionally, anytime you take classes at another institution, those credits should be certified with their VA Office.

PRE-REQUISITES:

You may not be within the last 30 hours of your program unless you have submitted an Academic Petition.

1.  Log on to Florida's Official Online Student Advising System

2. Click Transfer Services

3. Click Transient Student Form

4. Click Continue. It's not necessary to complete information on this page.

5. Select Home Institution

6. Enter your Student ID

7. Enter your PIN (Password)

8. Click Continue

9. Click Next

10. Select Program

11. Click Next

12. Complete Transient Student Form
