Application Instructions


Eligibility for Children:

Parent * a permanent & total service-connected disability, or
Must Have: * died as the result of a service-connected disability, or
  * died while totally & permanently service- connected
disabled, or
  * died in the line of duty while a service member


Eligibility for Spouses:

The veteran must have permanent and total service-connected disability.

Eligibility for Surviving Spouses:

The death of the veteran must be service-connected or veteran was permanently and totally service-connected disabled at time of death.

Next, you may complete the following forms with Florida Atlantic University:

1. Complete the VA Certification Request Form.
2. Complete Form 22-5490 – Application For Survivors' and
  Dependents' Educational Assistance.
3. Prior Education and Training Verification Form
