Returning to FAU after NOT using benefits for over 1 year |
If one of the above qualifications applies to your situation, complete the following forms with Florida Atlantic University:

Complete the VA Certification Request Form. |
| 2. |
Complete Form 22-1995 - Request for Change of Program |

or Place of Training. On the top of this form, please write on the top of this form that you are applying for Chapter 1607 benefits. |
| 3. |
Complete a Prior Education and Training Questionnaire |
| 4. |
Provide a copy of the DD-2384 - Notice of Basic Eligibility. |
| 5. |
Provide a copy of Certificate of Eligibility from the VA. |
| 6. |
Provide a copy of the DD-214 MEMBER 4. |
