If you are you a veteran who has been injured (Physical injury, PTSD, TBI etc) in Iraq or Afghanistan we would like to give you $20 to take a survey in order to help all veterans return to work

Directions and Informed Consent

Title: An investigation of the return to work of military personnel after injury.

Investigators: Dr. Michael Frain and Dr. Malachy Bishop

Purpose: The purpose of this study is to examine how employment and employment services change for individuals who acquire a disability during military duty.

Procedure: You will be asked a series of questions concerning work, as well as your experiences with deployment. Once you are finished with the questions, your information will be put into a computer database with no information that will make you identifiable. The survey should take approximately 20-30 minutes to complete.

Risks: There does not appear to be any significant risks to you in answering these questions.

Benefits: The benefits of this study may include information concerning how to best serve your return to work needs. You will also receive a $20 gift card of your choice after completing this survey, or we will donate the $20 to the Wounded Warriors Project.

Data Collection and Storage: All of the results will be kept confidential and secure. Only the investigators conducting the study will see the data, unless required by law.

Consent Statement:
