How to Submit Documents

1. Click on the document to be completed. Allow the form to appear on the screen.
2. Print out the form and complete.
3. Once the form is completed, scan the document and email it as an attachment to the FIAT Coordinator or deliver the document to the mailbox outside of Room 339, College of Education, Boca Raton campus

Participation Logs

Please print out a participation log to complete with each visit to your assigned school. You will submit this completed form to the Good FIT coordinator at the conclusion of your internship.
