PRECAUTIONS AT THE COMMUNICATION DISORDERS CLINIC

Client Screening Form

Thank you for your continued support of the Clinic! We have been actively seeking ways to protect everyone involved in the therapy and diagnostic processes of speech-language pathology and audiology. The health and safety of our clients is our prime concern; therefore, these measures have been implemented:

Arrival

The clinician will take your/your child’s temperature with a no-touch forehead thermometer before going into the therapy room. It is strongly recommended that clients wear facemasks. All Clinic Personnel will wear facemasks.

The completed attestation form needs to be e-mailed to jbayne1@health.fau.edu before your appointment, or can be handed to the clinician before services are provided.

Parents and guardians who transport children may accompany their child to the clinic and have the option of watching in the observation room or sitting in the waiting room or other location within the building. If you choose to observe the session, it is highly recommended that you wear a face mask and maintain proper distance from others observing.

The Physical Setting

Only the client and student clinician will be allowed in clinic rooms, which will allow for adequate (six feet) social distancing. Every effort will be made to keep a distance or a barrier between the clinician and client.

*Tissues, hand sanitizer, and disinfectant wipes are available in each therapy room for in-session use.

*All materials, chairs, and tables will be cleaned with disinfectant after each use.

Clinic Personnel

All personnel are required to wear a face mask and gloves, and be temperature-checked upon entry into the Clinic Area.

All personnel, if exposed to someone with a positive COVID status, will not be allowed in the Clinic until being tested, with negative results. If personnel test positive, they will be in quarantine for 14 days, test negative once and again test negative 3 days later.
