Most of the forms that you will need you can find at the Registrar’s Office website. From this webpage you can get forms for all of the following:

Graduation

Graduate and undergraduate students must submit an Application for Degree to graduate. This application must be submitted within the first couple of weeks of the semester that you will be graduating. Be sure to check the academic calendar for the submission date.

Change of Address

If you are moving be sure that FAU has your correct mailing address. Fill out the change of address form as soon as you know your new address.

Petitions

There are different petitions for graduate and undergraduate students. Read and follow the directions on your petition and consult with you academic advisor.

For undergraduate students only:

Undergraduate petition

Undergraduate Petition. This form is to be used for waiver of 9 credits in summer, course waiver, College requirement exceptions, concurrent enrollment, course equivalency/substitution for FAU courses (use Course Equivalent/Substitution Petition for courses taken at other institutions), waiver of last 30 hours in residence, change of sections during a semester, and other College requests.

Petition for a Late Withdraw

Petition for a Late Withdraw. This form is used for students requesting to withdraw from a class after the deadline. This form should be used in combination with the Instructor’s Memorandum.

Petition for Reinstatement following Academic Dismissal

Petition for Reinstatement following Academic Dismissal.  This form should be used for students who have been previously dismissed from the university for academic reasons (low GPA).  Please follow all directions on the petition, failure to do so will negatively impact any decisions.

Course Equivalency/Substitution Petition

Course Equivalency/Substitution Petition. This form is to be used by students requesting that a course from another institution be considered equivalent to or substituted for a FAU course. This is NOT to be used by architecture majors, please contact the advisor for the appropriate form.

Gordon Rule

If you have completed the Gordon Rule but your SASS report does not show it you should submit a Gordon Rule Evaluation.
