In keeping with the University's mission to improve and increase student retention, the Office of Freshman Academic Advising Services has developed this form so faculty can make us aware of first year students they deem to be in need of academic support services.

An overview of the information you provide will be shared with the student, and used by an academic advisor to assist the student in developing an "action plan" to address and remedy the academic or non-academic concerns.
